IC System, Inc. is a collection agency in St. Paul, Minnesota, and most of its work is medical. IC System says it has more than 4,000 healthcare clients, more than 70% of its client mix. Before you dispute: three checks on a medical bill
Medical collections often start from billing problems rather than unpaid care. Before writing to IC System, it is worth asking the provider for three things:
- An itemized bill, so you can see each charge, the date of service and any adjustments.
- Proof it was billed to your insurer, and your insurer's explanation of benefits for the same dates.
- The provider's financial assistance policy, if it is a nonprofit hospital (see below).

Nonprofit hospitals: financial assistance can still apply
Tax-exempt nonprofit hospitals must have a written financial assistance policy, must generally accept applications for 240 days after the first post-discharge bill, and must not take extraordinary collection actions such as credit reporting or lawsuits until at least 120 days after that bill.
If your bill is from a nonprofit hospital and you may have qualified for free or reduced-cost care, ask the hospital for its financial assistance application, even if the bill is already with IC System. If you apply inside the application window, the hospital has to consider it. IC System itself has written about these rules for its hospital clients.

Disputing with IC System
A written dispute inside the validation period obliges IC System to stop collecting until it mails verification. For a medical bill, our letter asks for the itemized balance from the itemization date and the name of the provider that owns the account.
IC System says it sometimes reports past-due accounts to the credit bureaus. If it has reported the account, ask it to mark the account as disputed. You can also ask IC System to stop calling and use mail only.

A worked example over Veterans Day
Say IC System mails a validation notice on Monday, November 9, 2026 for a $1,138.40 emergency room balance that you think your insurer should have covered.
- Veterans Day (November 11) does not count, so the fifth working day after mailing is Tuesday, November 17, 2026.
- The dispute period can end no earlier than Thursday, December 17, 2026.
- Ask the hospital for the itemized bill and your insurer for the explanation of benefits. If the hospital is nonprofit, ask for its financial assistance application too.
- Mail the dispute to IC System by certified mail, naming the hospital as the current creditor.
