Capio Partners: who owns your medical bill now

Capio Partners, LLC is not a typical collection agency. In 2022 testimony to Vermont lawmakers, Capio described itself as the largest buyer of healthcare receivables and said it does not resell accounts. So if Capio has written about a hospital or doctor's bill, the account may now belong to Capio or one of its companies, and the questions are whether the bill was right in the first place and what you can still claim from the hospital.

Why a Capio letter is about a hospital bill

Capio's website calls it a receivables management company "dedicated exclusively to medical accounts". Trade press reported Capio's launch in early 2008, as a company buying and collecting self-pay debt from hospitals. In 2025 it said it manages more than $51 billion in patient accounts receivable and partners with more than 1,000 healthcare providers.

A 2016 court filing shows Capio holding hospital account pools through affiliated companies such as CF Medical V, LLC, with a Capio affiliate collecting them for a fee. That is why the creditor on your notice may be a company you have never heard of. The notice must still name the current creditor and show how the balance was worked out. You can also ask in writing, inside the validation period, for the name and address of the original creditor.

Check the bill before you pay

Hospital bills that reach collections are often wrong or reducible. Before paying Capio, ask three questions:

  • Was insurance billed correctly? Ask for an itemized bill and the insurance payments and adjustments. A claim that was never filed, or was denied for a fixable reason, can wipe out much of a balance.
  • Did you qualify for financial assistance? Nonprofit hospitals must have a written financial assistance policy, and you can apply even after a bill has gone to collections. Ask the hospital for its policy and an application.
  • Is it yours and the right amount? A written dispute inside the validation period makes Capio stop collecting until it sends verification.

Capio says it does not charge interest or fees, so the balance should not have grown since the hospital sold it. If it has, ask why in your dispute.

Contacting Capio and paying safely

Capio's account line is 888-893-0171, and its website gives P.O. Box 120225, Dallas, TX 75312-0225 for complaints. You can ask for a Spanish translation of the first notice and to speak with a Spanish-speaking representative.

Capio tells people to check their letter for the right payment portal, or to call 888-893-0171 if they cannot find it. So check that any payment link matches the portal on your letter rather than one sent in a message. It says it will not try to contact you through social media. It offers a BuoyFi calculator for "affordable, income-based options that may lower your payments". Ask for any payment plan or settlement in writing before you pay.

Capio and your credit report

It says it reports medical debt to the credit bureaus only as the law and its agreements with provider clients allow. Under the credit bureaus' current policies, paid medical collections, medical collections under $500 and medical debts less than a year old do not appear on your report; our medical bills and credit guide explains the details.

If you dispute a debt and Capio reports it, it must mark it as disputed. In 2021 a federal court in Pennsylvania dismissed a consumer's FDCPA and credit reporting claims against Capio; the report says Capio had deleted the accounts from her credit file after her dispute.

Complaints and court cases

On October 11, 2026 the CFPB's public complaint database listed 3,857 complaints naming Capio Partners, 1,518 of them received since January 2025; the most common issue was an attempt to collect a debt the person said they did not owe. Complaints are what consumers report and are not checked by the CFPB, but the pattern is a reason to ask for proof.

An FDCPA suit filed against Capio in the Eastern District of Texas in April 2025 was settled and dismissed in July 2025. We found no CFPB or state enforcement order against Capio.

A worked example in April

Suppose Capio mails a notice on Wednesday, April 7, 2027 about a $1,964.30 emergency room bill from two years ago.

  1. Counting five working days, the assumed receipt date is Wednesday, April 14, 2027.
  2. The validation period runs until at least Friday, May 14, 2027.
  3. Ask the hospital for the itemized bill, the insurance claim history and a financial assistance application, and send Capio a written dispute asking for verification before that date.
